If you would like to receive dialysis treatment at a Renalis clinic, please fill in the visiting patient questionnaire. You can download the form here and send the completed questionnaire to renalis@renalis.ee
This questionnaire is intended for dialysis patients who are temporarily staying in Estonia and require treatment under the European Health Insurance Card. The form collects the necessary personal and medical information, as well as details about the period of stay in Estonia and the need for treatment.
Please include up-to-date treatment information, consent forms, and a copy of the European Health Insurance Card.
